Effective treatment of CLIPPERS with long-term use of rituximab

چکیده
Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) is a rare CNS inflammatory syndrome first described in 2010 by Pittock et al. Taieb et al. reported about 60 known cases throughout the world. The underlying pathogenesis of CLIPPERS has not been fully elucidated, and a neural autoantibody target has not been discovered. Steroid treatment is effective in treating the inflammatory component of the disease, but long-term treatment of CLIPPERS with steroids is limited by side effects. Steroid withdrawal is associated with recrudescence of the disease, and each relapse is associated with additional disability. Moderately efficacious steroid-sparing treatments include methotrexate, hydroxychloroquine, and cyclophosphamide. However, the long-term efficacy and safety of these agents in CLIPPERS remain to be determined. Anti-CD20 molecules benefit several antibody-mediated CNS inflammatory diseases. Herein, we describe a case of CLIPPERS successfully treated with rituximab (mouse chimeric anti-CD20 monoclonal antibody) over the course of 4 years. We describe imaging and histological features of this disease, as they are relevant to disease pathology and treatment (figure).

Efficacy of Rituximab in the Treatment of Plasma Exchange Refractory Thrombotic Thrombocytopenic Purpura
Background: Rituximab can induce a durable remission in plasma exchange refractory thrombotic thrombocytopenic purpura (TTP). Timing of Rituximab infusion in combination with plasma exchange (PE) and long term follow-up for probable side effects of such treatment is still lacking. Methods: This study was conducted among 10 patients with plasma exchange refractory TTP. According to the study de...

Background: Recently, rituximab has been successfully used for the treatment of pemphigus family, the main subtype of which is pemphigus vulgaris (PV). The aim of this study was to determine the efficacy and safety of rituximab in refractory PV. Methods: In an observational study extending from November 2014 to February 2016, 30 patients with refractory PV were treated with rituximab. Response ...

Background: Yazd has important potential role in treatment of infertility and promotion of sexual health in south of Iran. Long term usefulness of reversible contraceptives (LTURC) is the most cost-effective contraceptives between women. This study aimed to assess the reproductive aged women's knowledge, attitudes, and factors associated with use of LTURC. Materials and Methods: This cross...
متن کاملSuccessful Treatment of Refractory Thrombotic Thrombocytopenic Purpura with Rituximab, a Monoclonal Antibody: a Case Report
Thrombotic thrombocytopenic purpura (TTP) is mostly attributed to the presence of an autoantibody against ADAMTS-13, a metalloprotease that degrades ultralarge von Willebrand protein multimers. Accumulation of vWF multimers and systemic platelet aggregation lead to microangiopathic thrombosis, hemolytic anemia, and end-organ ischemia. Most patients respond to therapeutic plasma exchange (TPE), ...
متن کاملModerator's view: Should all patients with ANCA-associated vasculitis be primarily treated with rituximab?
Experience with rituximab in patients with new ANCA-associated vasculitis (AAV) is still very limited, especially in patients with severe (organ- or life-threatening) AAV. Rituximab may be more effective in anti-PR3 AAV, but potentially less effective in some granulomatous manifestations of AAV. We do not know what the response is to rituximab on the tissue level. Rituximab induction needs to b...
